My role is to help you understand and follow the care plan created by your provider, answer questions between appointments, and support you at home so nothing falls through the cracks. When needed, I can connect you with trusted Physicians who collaborate with Heart & Anchor.

  • Unlike traditional home care, Heart & Anchor is nurse-led, meaning your care is provided by a Registered Nurse, not a caregiver or aide. We offer clinical support, help coordinate with doctors, and guide you through recovery or health changes with personalized, flexible care. No checklist, no agencies, just expert support when you need it.
