Proposed Plan of Support
After your consultation, we outline recommended services, visit frequency, and area of focus based on your needs.
Services may be provided as individual visits or bundled care packages, depending on the level of support required. Plans remain flexible and evolve as your situation changes.
Step 4
Ongoing Support
Once services are initiated, nursing support is provided as outlined and adjusted as needs change. This may include in-home visits, coordination, monitoring, education, and ongoing communication with clients and families to support continuity of care.
